Distance Learning Technology Request Forms

Primary (Grades K-5)

All students will need a device to participate in our distance learning. If you are still in need of a device for your child for distance learning, please contact Media Specialist Leah Lieurance.

Secondary (Grades 6-12)

Parents of students in grades 8 through 12 who are unable to obtain their own device should email the appropriate grade-level Assistant Principal:
