Self-Screening Questions

Student Athletes must self-screen before EVERY PRACTICE, EVERY COMPETITION (even intramural competitions), and before every in-person meeting.
 
For after school and evening practices and competitions, student athletes must self-screen AGAIN just before the practice or competition, even if they attended school that day and self-screened at the start of the school day.
 
If a student begins to feel ill, parents should be prepared to return to school to pick them up immediately.
 
Questions:

1) Have you tested positive for COVID-19 since you were last on campus?

 ➤ If YES to Question 1, please remain home, quarantine, and consult a physician. 


You will be permitted to return to school once all four measures have been met:

  

  • You are 24 hours fever-free without using fever-reducing medication; and
  • Improved symptoms (cough, difficulty breathing, etc.), and
  • At least 10 days have passed since the symptoms began, and
  • A doctor’s note releasing you to return

2) Have you had close contact with any individual who is test-confirmed with COVID-19 within the last 14 days?

➤ If YES to Question 2, please remain home, quarantine for 14 days from the last date of close contact, and consult a physician. If you live with someone who is test-confirmed with COVID-19 and you cannot avoid continued close contact, you must quarantine for 14 days after the date the person with COVID-19 ends home isolation. 3) Have you experienced any of these symptoms?

  • Temperature 100.4 F or higher
  • New sore throat
  • Shortness of breath or difficulty breathing
  • New cough
  • New loss of taste or smell
  • Diarrhea or vomiting
  • New onset of headache

➤ If you have a fever, you should stay home. If you have symptoms that are a result of a noncontagious ailment, like seasonal allergies, use your judgement.

 

Information collected in these screens will not be retained if the screening is passed. If a parent or student indicates that the student is demonstrating COVID-19 symptoms, the school will retain that information until the student meets the criteria for return to school (and extracurricular activities).
