1) Have you tested positive for COVID-19 since you were last on campus?
➤ If YES to Question 1, please remain home, quarantine, and consult a physician.
You will be permitted to return to school once all four measures have been met:
2) Have you had close contact with any individual who is test-confirmed with COVID-19 within the last 14 days?
➤ If YES to Question 2, please remain home, quarantine for 14 days from the last date of close contact, and consult a physician. If you live with someone who is test-confirmed with COVID-19 and you cannot avoid continued close contact, you must quarantine for 14 days after the date the person with COVID-19 ends home isolation. See CDC guidance.
3) Have you experienced any of these symptoms?
➤ If you have a fever, you should stay home. If you have symptoms that are a result of a noncontagious ailment, like seasonal allergies, use your judgement.
Information collected in these screens will not be retained if the screening is passed. If a parent or student indicates that the student is demonstrating COVID-19 symptoms, the school will retain that information until the student meets the criteria for return to school (and extracurricular activities).
