Transcript Requests

Current Seniors

- Common App transcript requests should be made directly to the counselor:
1) Select a college from your list, then open the Recommenders and FERPA section
2) Complete the FERPA agreement, and then check "I waive my right to review all recommendations and supporting documents".
3) Click on Invite Counselor and choose either Mrs. Gregg ([email protected]) or Mr. Manuszak ([email protected]), then click on "Enable Preview" to complete the request.

 

- For all other colleges/universities, scholarships, or other organizations that are not part of Common App, please complete a Senior Transcript Request Google Form

 

 

 

 

Current 9th-11th Graders

For 9th-11th grade transcript requests for summer programs, employment, or other purposes, please email [email protected] and include all necessary details.

 

Meridian alumni

For alumni transcript requests for colleges/universities, employment, or other purposes, please email Requests and include all necessary details.

 

IB Exam Transcripts

IB exam transcripts are $18 and can only be requested from the IBO. They are available after students receive their IB exam results in early July.

  1. Read details/instructions.
  2. Fill out this form. Choose the option that says "Request for results to Institutes of higher education" if you are requesting an IB transcript to be sent to a university.

 

 

Verification of Enrollment (VOE) for Driver Permit/Driver License Appointments

To request Verification of Enrollment (VOE) Form for DPS (for driving purposes), please complete this VOE Request Form. It takes up to five school days to process a VOE, so please plan ahead, especially if your DPS appointment is scheduled during a break. Questions about VOE? Email Karina Coburn.

 

 

Other Document Requests

To request any other type of document such as report cards, official letters, immunization records, insurance verification, letters for court, etc., please email Requests and include all necessary details.

To report a change to home address and/or mailing address, please email Registration.
