Medication Consent Forms and Care Plans

Medication Consent Forms are required for all medications administered at school and on school trips.

  • "Medications" also include any over-the-counter medications. Per Texas Board of Nursing, the school requires a doctor's order to administer these.
  • Medication Consent Forms and Care Plans do not carry over from one year to the next; a new form must be submitted to the nursing team each year.
  • Please download, complete, and bring the appropriate form with your child's medication in the original prescription container (e.g. not in a Ziploc bag).
