Medication Consent Forms and Care Plans
Medication Consent Forms are required for all medications administered at school and on school trips.
- "Medications" also include any over-the-counter medications. Per Texas Board of Nursing, the school requires a doctor's order to administer these.
- Medication Consent Forms and Care Plans do not carry over from one year to the next; a new form must be submitted to the nursing team each year.
- Please download, complete, and bring the appropriate form with your child's medication in the original prescription container (e.g. not in a Ziploc bag).

Self Carry Medication Form for Secondary Students (6-12) for the following medications:
- Epi-Pen for allergic reactions
- Inhaler for asthma
- Medications for diabetes
Care Plans should be shared each year with the Nurse's office.

Seizure Action Plan and Diabetic Care Plan: your physician will generate this for you.
