A valid physical form must be on file with the Athletics Department before students can participate. 

Email the completed physical form to [email protected]. Important! No hard copies will be accepted!
 
Students must have a valid physical form on file with the Athletics Department prior to participating.
  1. Check the expiration date of your physical form.
    • If your name is not listed, it is because we do not have a physical form on file for you.
    • If you see "pending" next to your name, it is because the physical form submitted cannot be processed as-is. Please email [email protected] to inquire.
    • If you recently submitted your physical form but your name is still not listed or your physical form's expiration date has not yet been updated, please check the date and time the physical form sheet was last updated (at the top). If it has been updated since you submitted your physical form, please email [email protected] to inquire.
  2. Find a blank 2026-2027 physical form to complete.
(The 2026-2027 form can be used for students who need to update their physical form during the 2025-2026 academic year as well as during the 2026-2027 academic year. Physical forms are valid for one year from the date of the doctor's examination.)
  1. Print and complete the physical form.
    • If you are unable to print a blank physical form, please stop by the middle school or high school office and request a blank physical form.
    • Tips for submitting a valid physical form: In order for a physical form to be approved by the Athletics Department, all of the following must be true:
      • It must be filled out completely.
      • The Medical History side must be completed by the student athlete and signed and dated by the student athlete and a parent/guardian.
      • The Physician's Examination side must be completed by a physician who is licensed in the state of Texas. It must indicate that the student athlete is physically "cleared" to participate. It must include the physician's name, address and phone number. It must include the physician's signature. It must include the date of examination. All of which must be legible.
        • Physical forms are valid for one year from the physician's examination date.
  2. Submit completed physical forms to [email protected]. Please do not submit paper physical forms. They are needed electronically to process.
 
Q: When are physical forms due?
A: Each sport has its own physical form due date based on its season. Physical forms are usually due the same date the sport's registration closes. Please check your sport's webpage for Important Dates, including when physical forms are due.
 
Physical forms are processed once weekly. Please plan accordingly. Missing a physical form deadline could mean that your student does not establish physical eligibility in time for tryouts/the start of a sport's experience. Additional tryout dates are unavailable.
 
Physical forms for 2026-2027 Dance Team, Flag Football, and Volleyball are due April 23. Physical forms for 2026-2027 Cross Country are due August 1. These sports web sections will be updated a week before registration opens.

Q: Where do I get my child's sports physical done?
A: Sports physicals or physical examinations can be given during a well child check or routine annual examination. Be sure to ask the physician to complete the physical form while you are there. Sometimes, a family doctor or pediatrician will provide a physical form based on an earlier routine examination. Remember, physical forms are valid for one year from the date of the physician's examination (not the date the form was completed or submitted). Urgent care facilities also offer sports physicals.
